All documents are required at the time of registration. Proof of income and identification are required to determine income tier and eligibility to receive services. More information provided in the forms that can be accessed below.
Disclaimers
We will see patients on a first come, first served basis. We understand your time is valuable and so is ours. We want to be sure all of our patients get fair and equal treatment. We do have a no show policy that if you do not show up when your turn is called 2 visits in a row, you will no longer be eligible to use our service.
We do not carry prescriptions or cash on site.
We are not a primary care or follow up care facility. We do not carry prescription medications on site, and we will not prescribe Schedule 2 medications. If you have a condition that requires higher level medications, please see your doctor or the Emergency Department.
We do not accept any form of insurance, Medicaid, or Medicare.
